GA Pre-K Documentation Requirements
At the beginning of every school year, we are tasked to provide and collect the following information to be kept in your child's student record in accordance with Bright From the Start's Pre-K Guidelines. Please note the documents below are needed in your child's permanent record to complete their Pre-K registration and not jeopardize your child's Pre-K seat.
- A copy of the child's original CERTIFIED birth certificate
- Proof of Residency (A gas, electric or water bill, contract, or lease agreement that is current within a month of the first day of school or after .)
- Bright from the Start Roster Information Form ( BFTS_Roster_Info.pdf )
- Official Pre-K Registration Form (2023 Registration Form will be provided by teachers if needed)
- Category I proof, if you receive any of the following (Copy of only 1: CAPS, Food Stamps, Medicaid, SSI or TANIF)
- The Child's Social Security Card or a completed SSN Waiver Form ( BFTS_SSWaiver.pdf ) (Spanish Form)
- Health immunization form 3231 or a notarized Affidavit of Religious Objection to Immunization (DPH Form 2208) is presented.
- Note: This must be on file within 30 days of the child starting school or updated if the form expires during the current year. If it is not provided, the child will not be able to return to class without the form or proof of an appointment. Should the child be out for 10 consecutive days due to this form not being provided the child's spot in pre-k will be forfeited.
- Health form 3300 which shows evidence of completed hearing, vision, dental, and nutritional screenings ( Dated no more than 12 months prior to the start of the school year. )
- Note: This must be on file within 90 days of the child starting school or updated if the form expires during the current year. If it is not provided, the child will not be able to return to class. This may result in the child losing their spot in the program.
