-Form 3231I Georgia Immunization Record
-Form 3300 Georgia Vision Hearing Dental Screening (for 3 year olds, if pediatrician does not test vision and hearing until age 4, we will still need the signed form with a written statement about the reason for a lack of test results).
-Original Birth Certificate
-Social Security card or completed waiver
-School Registration packet and medical packet
-Guardianship or Custody Orders, if applicable
